This is PRE-DETERMINED by your employer or referring agency.
If you have a Misc. No. (Billing Number) you will only need to pay the "Rolling Fee!"

Colleges, Schools, Universities, and Businesses.
Groups of 6 persons or more are required.
Group Rates and Billing are provided for your convenience.
